Stages of Alzheimer's Disease: Subtle Behavioral Shifts to Monitor in Aging Parents

Early Stage: Subtle Indicators

In the initial phase of Alzheimer's disease, cognitive deficits are mild and often mistakenly attributed to normal aging:

  • Short-Term Memory Lapses: Repetitive questioning within short intervals and forgetting recent conversations (while long-term memories remain intact).

  • Visuospatial and Temporal Disorientation: Hesitation in familiar environments or confusion regarding dates and seasons.

  • Executive Function Decline: Struggles with managing finances, paying bills, or completing multi-step daily tasks.

Middle Stage: Disease Progression and Functional Decline

Damage spreads from the hippocampus to broader cortical networks:

  • Prosopagnosia & Recognition Deficits: Difficulty identifying familiar acquaintances or extended family members.

  • Loss of Instrumental Activities of Daily Living (IADLs): Challenges in selecting weather-appropriate attire, maintaining hygiene, or preparing meals independently.

  • Behavioral Shifts: Mood lability, unprovoked paranoia, anxiety, or late-afternoon agitation (sundowning).

Late Stage: Advanced Dependence

Severe cortical atrophy leads to loss of verbal communication, motor coordination, and complete dependence on round-the-clock care.
